Tackling the Challenge of Delayed Bleeding After ESD: New Advances in Hemostatic Techniques and Clinical Practice

The Key Problem: Delayed bleeding post-ESD is a critical clinical pain point. 

Endoscopic submucosal dissection (ESD) is a cornerstone minimally invasive technique for treating superficial gastrointestinal tumors. With an incidence rate ranging from 1.8% to 15.6%, DB is influenced by factors such as patient age, lesion size and location, and the use of antithrombotic agents. When it occurs, DB often necessitates emergency hospitalization, potentially requiring blood transfusion or repeat endoscopic intervention, thereby increasing patient morbidity and healthcare resource utilization. Consequently, the exploration of safe and highly effective post-procedural hemostatic strategies has become an urgent priority in the field of gastrointestinal endoscopy.

Academic research: Multi-center data validates the value of hemostatic powder

To evaluate the efficacy of hemostatic powder in preventing delayed bleeding after ESD, a multicenter retrospective study in the United States enrolled 83 patients (excluding those undergoing mucosal defect closure) who received the hemostatic powder after ESD at three clinical centers between January 2023 and March 2024. The patients had a median age of 66 years, and their lesions were located in the esophagus, stomach, colon, and rectum.

Results showed that after using hemostatic powder, only 3 patients (3.6%) experienced delayed bleeding after surgery, all of which occurred within 24 hours and stopped spontaneously, without the need for secondary endoscopic intervention. This suggests that effective hemostatic products must be easy to use, provide stable coverage, and be safe and reliable to meet the hemostatic needs of complex wounds after ESD surgery.
